LETTER FROM SOUTHEAST COMMUNITY HEALTH SYSTEMS
Dear Parents,
Letters are being sent home this week and next week regarding students with overdue immunizations. Several immunizations are required before attending school, unless a statement of exemption from immunization form has been completed by the student’s pediatrician.
Southeast Community Health System has a school-based health center on each site, staffed with nurse practitioners, where these immunizations can be given safely and conveniently while your student is at school. This is available free, with the Southeast nurses consent packet and a separate immunization consent.
The Southeast nurse consent packet will be sent home with your student over the next several days and is needed for you student to see the nurse for any reason. This packet must be completed each school year. The consent packet is also available at the school’s front office, each school’s nurses station, and online at the sthpk-12.net under the “Documents” section.
If your student has overdue immunizations, the separate immunization consent forms will be attached to your child’s letter. Students should return all consents to their homeroom teachers or bring directly to the nurses station.
